Hope I explain this properly.

With runners on first and second, a RHP turns towards first on his way to bluffing a throw to try to pick the runner off second.

Shouldn't he have to turn towards the 3rd base side when trying to pick off the runner at 2nd? (with a runner on first)

It seems that when he spins towards first he should have to throw over there.

No. He didn't stop and fake to 1B, but continued his move toward 2B.

Now, take a right-handed pitcher's stance, stretch, and try to turn to your right (toward 3B), and fake to 2B. Try to avoid falling while doing so. This is extemely awkward.
