Any additional defensive conferences require removal of the pitcher. Ok you have a starting pitcher and you make a trip in the first, second and third inning. So you make the pitching change in the third inning - now do you have another three trips since it's a new pitcher OR do you have to make a change on your first trip (fourth overall) to the new pitcher? I have always wondered about this. Thanks

You get 3 trips per GAME. After the 3rd charged conference, every visit requires a pitching change. In your example, the 3rd visit wouldn't be charged because you changed the pitcher. You would still have one left.
