It's kind of hard to know without actually seeing you pitch. How ever there are a couple things. One thing is check your arm slot. Your arm slot could be causing you to never get ontop. Another thing it could be is where you release the ball. Make sure to be reaching out front when your pitching. If your throwing the ball back you will never get leverage on top of the ball. Last thing you should check is just, where you are positioning your wrist and how you move it. I know sometimes when I pitch I might sumtimes go around the ball, by wrapping around the ball rather than coming on top and snapping down.

You are wise to correct this and to listen to your body when it is talking to you. Focus on having your wrist hinge, rather than twist. It is something you need to work on EVERY TIME you throw the ball, whether in warmups, in the field, in the bullpen or on the mound. Good luck.
